Graduation Audit Request Form

PURPOSE

The purpose of this form is to allow you to graduate as a member in good standing of the Justice Action Center. By completing this form, you are indicating to the Center how you will have completed the various requirements of Center membership prior to your graduation. Please note that this is separate and distinct from your general graduation requirements.
