We have collected below the forms and resources most frequently requested by students. (All links are either to web-based forms or PDFs.) If you have any difficulty printing out the forms, please stop by the center (on the 9th floor of 40 Worth) to pick up a hardcopy.

Completing Your Center Requirements

• Current Curricular Requirements
  This document includes a complete list of the center's current (2012) requirements, as well as a list of faculty and pre-approved concentrations. (Students who affiliated in the spring of 2011 should reference that year's curricular description, available here. Students who affiliated in the spring of 2010 should reference that year's curricular description by clicking here.)
• Concentration Change
  All students must complete a concentration in a focused area of study. Students who would like to either change their current concentration or propose a student-designed concentration must complete this form.
• Colloquium: Most Recent Syllabus
• Social Justice Placement: Approval Form
  This form should be completed and submitted before you begin your placement.
• Social Justice Placement: Completion Form
  This form must be submitted after you have completed your placement.
• Capstone: Most Recent Syllabus
  The capstone syllabus varies according to how students choose to fulfill this requirement. Students complete one of the following:
  • JAC Capstone Course
  • Clinic
  • Project-Based Learning Course
  • Law Review Note
• Graduation Audit Request Form
  This form must be completed at the beginning of your final year of study. It is required for the Center to certify that you have completed all requirements prior to graduation.
